A note is text you add: what a user told you, a summary, anything worth remembering. A title is optional.

A title is optional. When you give one, each line `context()` writes from that note cites it, as
`[GTM plan, 2026-10-05]`; a note without one is cited as `[note, 2026-10-05]`.
